Proof of your citizenship or residency status

AllÌýapplicants applying to the MDCM or joint programs are required to submit proof of residency for their required applicant cohort (International, Canadian, or Quebec resident)Ìý Ìý

±·´Ç³Ù±ð:Ìýapplicants to the Med-P Qualifying YearÌýareÌýnotÌýrequired to submit such information unless specifically requested (e.g. presented in the application checklist in the online application system).

Residency for MDCM admission purposes is established as of the program application deadline. A change in residency after this date will not result in a change of applicant category. Residency documents are reviewed by the Undergraduate Medical ɬÀï·¬ office, who will make a determination of Residency based on the validity of the documents submitted.Ìý

Applicants whose documents do not fulfill residency requirements for the applicant category they have selected will be notified and switched to the appropriate category.

Note: Residency for tuition fee purposes is reviewed separately by Enrolment Services and is established based on the admitted student’s residency situation as of the start of classes.

Format and Submission

The final document should be in a single PDF file uploaded to the designated checklist item of your application via the online application system.ÌýÌý

Note that only Canadian citizens (including Canadian First Nations and Inuit) and Permanent Residents of Canada can claim Quebec residency.Ìý To be considered a Quebec resident as outlined by the Quebec government you must be able to prove your residency status according to one of the following 14 situations. Ìý

IMPORTANTÌýâ–º Quebec residency status must beÌýfullyÌýestablished (not provisionally) by the application deadline of November 1st for all those applying to the MDCM program. Changes of citizenship or residency after the application deadline will not be considered.

Situation 2

You are a Quebec CEGEP student entering ɬÀï·¬, and you cegep previously determined that you have Quebec resident status.

Applicants for the Med-P Qualifying YearÌýdo not need to provide proof of Quebec residency unless requested. Proof for one of the other situations will be requested if you were not considered a Quebec resident by your cegep.

Situation 3

You are coming to ɬÀï·¬ from another Quebec University where you were considered a Quebec resident, and have not interrupted your studies for more than two semesters (not including the summer semester).

Only select this situation if no other situation applies. For example, you may use the same situation that you used when you obtained Quebec residency at the other Quebec institution.ÌýÌýAdditional documentation may be required, and assessment may be delayed as this status must be validated by the Ministry of Education.

You had Quebec residency status in university and are continuing on at anotherÌýuniversity without interrupting your studies for more than two semesters (not including the summer semester)..

ÌýProof of Canadian citizenship orÌýCanadian permanent residencyÌý

If you are coming directly to ɬÀï·¬ from another Quebec university where you were already a proven Quebec resident, we will normally not require any documentation from you because we can requestÌý that your information be transmitted to us directly by the Ministry of Education.ÌýNote that this situation may not be used by Special or Visiting students for a Summer term.

Situation 4

You held a Quebec Selection Certificate (CSQ) at the time you became a Canadian permanent resident

1)ÌýÌýÌýÌýÌýÌý Proof of Canadian citizenship or Canadian permanent residency *Ìý Ìý Ìý

2)ÌýÌýÌýÌýÌý AÌývalidÌýCSQÌýby way of one of the following:

  • a copy of your CSQ
  • a copy of your IMM landing paper upon which your CSQ number is machine typed
  • a copy of your parent's CSQ if your name appears on their CSQ
  • a copy of your parent's CSQ and your parent's IMM landing paper if your name appears on their IMM landing paperÌýÌýÌý
  • an official letter from theÌýMICCÌýattesting that youÌýÌýwere previously issued a CSQ

*ÌýCitizensÌýare not required to demonstrate a landing date.

Situation 5

Your parent's [3] (or sponsor's) main residence is in Quebec

1)ÌýÌýÌýÌýÌý Canadian birth certificate [6];ORÌýproof of Canadian citizenship or Canadian permanent residencyÌý°¿¸éÌýÌýaÌýcertificate of Indian statusÌý°¿¸éÌýaÌývalid .ÌýÌý

AND:

2)ÌýÌýÌýÌý A copy of a government-issuedÌýdocument validating kinship between you and your parent orÌýÌýÌý

AND:

3)ÌýÌýÌýÌý Parent/sponsor's proof of Quebec residency. Either a copy of their validÌý[4]-issued QuebecÌýMedicare cardÌýORÌýproof that your parent's or sponsor's main domicile is in Quebec

Situation 6

You resided in Quebec for one year preceding the application deadline and did not study full-time at a Quebec educational institution

1)ÌýÌýÌýÌýÌý Canadian birth certificateÌý[6]ORÌýproof of Canadian citizenship or Canadian permanent residency Ìý°¿¸éÌýÌýaÌýcertificate of Indian statusÌý°¿¸éÌýa validÌý.ÌýÌý

AND:

2)ÌýÌýÌýÌý AÌýsworn statement, wherein you attest that you were not a full-time[5] student in the province of Quebec during the relevant 12-month reference period. Note the statement must be signed by the commissioner of oaths for the purposes of Medicine ɬÀï·¬.

AND:

3) For this 12-month reference period: either a copy of a validÌý4-issuedÌýQuebecÌýMedicare cardÌýorÌýproof that your main domicile is in Quebec.

Situation 7

You are receiving financial assistance fromÌýÌý(AFE)

Or you are the former recipient and did not interrupt your studies for more than two consecutive semesters (not including the summer semester).

1)ÌýÌýÌýÌýÌý Canadian birth certificateÌý[6]ORÌýproof of Canadian citizenship or Canadian permanent residencyÌý°¿¸éÌýÌýaÌýcertificate of Indian statusÌý°¿¸éÌýaÌýa valid .ÌýÌý

AND:

2)ÌýÌýÌýÌý Proof of receipt of Quebec financial aid. Proof can be demonstrated by submitting the confirmation letter from AFE (financial assistance statement)ÌýORÌýa copy of your certificate of guarantee delivered by AFE.Ìý

AND:Ìý

3)ÌýÌýÌýÌý If this will be your first program at ɬÀï·¬ and you have received aid previously while registered at another Quebec university, copies of your previous school's transcripts (N.B. Only transcripts from a post-secondary school situated in Quebec are acceptable)

Situation 8

You have lived three months in Quebec without having resided elsewhere in Canada for more than three months

One of 3 scenarios must apply:

1)ÌýÌýÌýÌý Permanent Resident (landed immigrant) You are a permanent resident of Canada who has taken up residency in Quebec within the firstÌýthree months after your permanent resident landing date (i.e., the date you became a permanent resident). Provide:

a.ÌýÌýÌýÌýÌý Proof of Canadian permanent residency.ÌýÌý

AND:

b.ÌýÌýÌýÌýÌý Proof of your Quebec residency since your arrival in the province. Either a copy of your validÌý4 Ìýissued QuebecÌýMedicare cardÌýorÌýproof that your main domicile is in Quebec.

OR,

2)ÌýÌýÌýÌý Returning Permanent Resident (landed immigrant) You are a permanent resident of Canada, who departs from Canada within the first three months of your date of landing and subsequently establish yourself in Quebec immediately upon your return to Canada. Provide:

i.ÌýÌýÌýÌýÌý Proof of Canadian permanent residency.

Ìý AND:

ii.ÌýÌýÌýÌýÌý Proof of your activities outside of Canada for the period between when you first landed in Canada and when you returned to live in Quebec (e.g., enrolment letter from former school, academic transcripts clearly indicating range of dates of enrolment, proof of employment or residence).

AND:

iii.ÌýÌýÌýÌýÌýÌý TheÌýswornÌýstatement for returning permanent residents.ÌýNote the statement must be signed by the commissioner of oaths for the purposes of Medicine ɬÀï·¬.

AND:

iv.ÌýÌýÌýÌýÌý Proof of your Quebec residency since your arrival in the province: Either a copy of your validÌý4Ìýissued QuebecÌýMedicare cardÌýorÌýproof that your main domicile is in Quebec.

OR

3)ÌýÌýÌý Canadian citizen (Including Canadian citizens born abroad) ÌýYou are a Canadian citizen living in Canada for the first time in your lifetime. Before entering Quebec, you never resided in another province or territory than QuebecÌýfor more than three cumulative months. You have now been living in Quebec for at least three months.

i.ÌýÌýÌýÌýÌý EitherÌýaÌýCanadian birth certificateÌý[6]ORÌýProof of Canadian citizenship.

AND:

ii.ÌýÌýÌýÌýÌý Proof of activities for the 10-year period prior to your arrival in Quebec (e.g., enrolment letter from former school, academic transcripts clearly indicating dates of enrolment, proof of employment and/or residence)

AND:

iii.ÌýÌýÌýÌýÌýÌý ÌýTheÌýsworn statement for Canadian citizens.ÌýNote the statement must be signed by the commissioner of oaths for the purposes of Medicine ɬÀï·¬.

AND:

iv.ÌýÌýÌýÌýÌý Proof of your Quebec residency since your arrival in the province. Either a copy of your validatedÌý4 issued QuebecÌýMedicare cardÌýorÌýproof that your main domicile is in Quebec.

Situation 9

Your spouse is considered to be a Quebec resident according to one of the 13 other Quebec residency situations

Either aÌýCanadian birth certificateÌý[6]ORÌýproof of Canadian citizenship or Canadian permanent residency ÌýORÌýaÌýcertificate of Indian statusÌýORÌýÌýa valid .

AND, documents for 1 of these 3 scenarios:

1.ÌýÌýÌýÌý Legal Spouse by Marriage or Civil Union

a.Ìý Ìý Ìý A copy of your marriage or civil union certificate.

AND:

b.Ìý Ìý ÌýAll of the documents for your spouse meeting one of the 13 other situations available to prove that they are a resident of Quebec, as well as their proof of Canadian status (Canadian birth certificate [6]ÌýOR proof of Canadian citizenship or Canadian permanent residency OR a certificate of Indian status OR a a valid .)

OR,

2.ÌýÌýÌýÌý Common-law Spouse with child(ren)

a. Ìý Ìý ÌýTheÌýsworn statementÌýattesting toÌýoneÌýyear of cohabitation as proof of your common-law relationship with your spouse.ÌýNote the statement must be signed by the commissioner of oaths for the purposes of Medicine ɬÀï·¬.

AND:

b. Ìý Ìý A copy of the birth (or adoption) certificate for your or your spouse's child. It mustÌýstateÌýthe names of both parents of the child.

AND:

c. Ìý Ìý All of the documents for your spouse meeting one of the 13 other situations available to prove that they are a resident of Quebec,Ìýas well as their proof of Canadian status (Canadian birth certificate [6] OR proof of Canadian citizenship or Canadian permanent residency OR a certificate of Indian status OR a a valid .)

OR,

3.ÌýÌýÌýÌý Common-law Spouse without child(ren)

a. Ìý Ìý ÌýTheÌýsworn statementÌýattesting toÌýthreeÌýyears of cohabitation as proof of your common-law relationship with your spouse.ÌýNote the statement must be signed by the commissioner of oaths for the purposes of Medicine ɬÀï·¬.

AND:

b. Ìý Ìý All of the documents for your spouse meeting one of the 13 other situations available to prove that they are a resident of Quebec,Ìýas well as their proof of Canadian status (Canadian birth certificate [6] OR proof of Canadian citizenship or Canadian permanent residency OR a certificate of Indian status OR a a valid .)

Situation 10

You are a member of a First Nation or Inuit nation in Quebec

Consult the Quebec Ministry of Education's list ofÌýrecognized nations

Members of a First Nation in Quebec

1)ÌýÌýÌýÌýÌýÌý Your validÌýIndian status cardÌýissued by the federal government of Canada (both sides of card). Your card must indicate that you are a member of a recognized Quebec territory

If your card does not indicate a recognized Quebec territory, you must also provide one of the following:

2)ÌýÌýÌýÌýÌý Band council letter. A letter signed by your band council and on band letterhead, attesting that you are a member of a Ministry-recognized Quebec nation. OR,

3)ÌýÌýÌýÌýÌý James Bay Beneficiary card.Ìý If you are a James Bay Treaty beneficiary, a copy of your beneficiary card issued by the Cree Grand Council, as long as your beneficiary number is shown.

Inuit beneficiaries of the James Bay and Northern Quebec Convention

1)ÌýÌýÌýÌýÌý Your validÌý.Ìý

If your card does not indicate a recognized Quebec territory, you must also provide:

2)ÌýÌýÌýÌý a letter from the Makivik Society confirming that you reside in Quebec

Situation 11

You continued to reside in Quebec even though your parents or sponsor no longer reside here.

* You cannot have leftÌýand then returned to Quebec since their departure.

1)ÌýÌýÌýÌýÌý Either aÌýCanadian birth certificateÌý[6]ORÌýproof of Canadian citizenship or Canadian permanent residencyÌýORÌýaÌýcertificate of Indian statusÌýORÌýa validÌý.

AND:

2)ÌýÌýÌýÌý A copy of a government-issuedÌýdocument validating kinship between you and your parents ORÌýyour CanadianÌýpermanent resident landing paper which clearly indicates the name of your sponsor.

AND:

3)ÌýÌýÌýÌý Proof that your principal and permanent domicile has been in Quebec since your parents or sponsor stopped residing in Quebec. ÌýDocuments must be valid from the time your parent(s) or sponsor stopped residing in Quebec. ÌýÌýÌýEither a copy of your validÌý4-issuedÌýQuebecÌýMedicare cardÌýorÌýproof that your main domicile is in Quebec.

AND:

4)ÌýÌýÌýÌý Proof of Quebec residencyÌýof one of your parents/sponsor up until their date of departure from the province.

Situation 12

You wereÌýadopted by a person residing in Quebec at the time of your adoption.

1)ÌýÌýÌýÌýÌý Either aÌýCanadian birth certificateÌý[6]ORÌýproof of Canadian citizenship or Canadian permanent residencyÌýORÌýaÌýcertificate of Indian statusÌýORÌýaÌý.

AND:

2)ÌýÌýÌýÌý A copy of the adoption order issued by a Quebec courtÌý

Situation 13

Your two parents are deceased and one of your parents or your sponsor was residing in Quebec at the time of death

Both of your parents must be deceased, but only one of the parents must have hadÌýpermanent domicile in Quebec at the time ofÌýdeath.

1)ÌýÌýÌýÌýÌý Either aÌýCanadian birth certificateÌý[6]ÌýORÌýproof of Canadian citizenship or Canadian permanent residencyÌýORÌýaÌýcertificate of Indian statusÌýORÌýaÌý.

AND:

2)ÌýÌýÌýÌý A copy of a government-issuedÌýdocument validating kinship between you and your parents.

AND:

3)ÌýÌýÌýÌý A copy of theÌýdeath certificate for each of your parents or sponsor (at least one of which was issued by theÌýDirecteur de l'état civil du Québec).

Situation 14

You have already been granted Quebec resident status in the past and in the past 5 years:

1)ÌýYou were enrolled at a Quebec post-secondary institution; andÌý

2)ÌýYour tuition status at that Quebec post-secondary institution during your period of enrollment was that of a Quebec resident; and

​3) You were residing in Quebec for at least 3 consecutive years.

Only select this situation if no other situation applies. Additional documentation may be required, and assessment may be delayed as this status must be validated by the Ministry of Education.

1)ÌýÌýÌýÌýÌýÌý Canadian birth certificateÌý[6]ÌýORÌý proof of Canadian citizenship or Canadian permanent residencyÌýOR Certificate of Indian statusÌý°¿¸é ÌýaÌý.

AND:

2)ÌýÌýÌýÌýÌý QuebecÌýÌýissued Quebec Medicare cardÌý°¿¸éÌýproof that your main domicile is in Quebec; must be valid forÌýthree consecutive years within theÌýfive-year reference period immediately preceding the MDCM application deadline.



[1]ÌýQuebec birth certificate: Your Quebec birth certificate must have been issued by theÌýDirecteur de l'etat civil, and must feature the notationÌýCertifié conformeÌýor Certified. The certificate must have been issued after January 1994. Certificates markedÌýCertifié conforme a l'article 137Ìýare only acceptable to prove Canadian status, and not Quebec residency.Ìý Long-form Birth Certificates are preferred, but Short-form is acceptable if all necessary information for your selected Situation is included. If you were born before 1994 you can provide a Statement of live birth or birth registration certificate.Ìý Statement of live birth or birth registration certificates issued by the Quebec government must include the place of birth on the document in a clearly identifiable location in Quebec, e.g., Montreal. Statements of live birth that have been noted with the following are not acceptable: "This document is not a birth certificate" or "this document is not an act of birth".

[2]ÌýCanadian Passport: A valid or expired Canadian passport is acceptable where the place of birth indicated is a clearly identifiable location in Quebec AND you already have aÌýQuebec permanent codeÌýwhich has beenÌývalidatedÌýto ɬÀï·¬ by the Ministry of EducationÌý(i.e. the perm code is visible in your Minerva account at Student>Student Accounts>View Tuition & Legal Status).

[3]ÌýParent/ sponsors:
a) Your parent/sponsor'sÌýprincipal and permanent residenceÌýmust be in Quebec.
b) Your parent/sponsor is not required to reside in Quebec for a minimum time before you may benefit from Quebec tuition rates.
c) You are not required to reside with your parent/sponsor.
d) You do not have to be a minor.
e) A 'sponsor' refers to a Canadian citizen or a permanent resident other than the student's father, mother, parent, spouse or partner, who sponsors the application for settlement in Canada under the Immigration and Refugee Protection Act (SC 2001, Chapter 27).
f) A Legal Guardian recognized by a federal or provincial court is also considered as your parent. Proof is requried.

[4]ÌýÌýRAMQÌýCard-issuedÌýQuebecÌýMedicare card.ÌýExpired cards are not acceptable.Ìý

[5]ÌýFull-time studyÌýis defined as being registered for 12 or more credits in one semester, or for an activity which is considered equivalent to full-time status. (e.g. some internship or co-op programs, thesis full-time, thesis additional session, thesis evaluation, or non-thesis extension). Note that grades of W count towards your status as full-time.

[6]Note that a provincial "Statement of Live Birth" is not accepted, the Canadian Birth certificate is required. Contact your provincial Service office or Vital Statistics office to obtain this document.

Applicants are expected to conduct themselves according to the terms of theÌýFaculty of Medicine Code of ConductÌýwhich apply to learners. The submission of false, incomplete, inconsistent or misleading information, or any omission that may result in a false or misleading conclusion, constitutes misconduct in the admission process. A finding of misconduct in the application process may lead to a refusal or, if an offer of admission has already been extended, a withdrawal of the offer at the sole discretion of the University.
