Applications are accepted year-round. Please note that externships are usually booked six months in advance.
The following documents are required, and should be combined into a single PDF document:
- a letter from your dental school indicating that you are in good standing,听or if you are听in a听residency program听a letter from the Program Director;
- your current curriculum vitae;
- immunization history;
- the completed听
OMFS Externship Application - a photo or headshot of yourself.
